Prepare your visit
Make sure you have your medicare card with you, and that it is valid (not expired), otherwise there would be a fee to pay!
Make sure you update your contact information (address, phone, email etc) with our secretariat so that we can reach you in case of need.
Bring your list of medications to day. It can be provided by your pharmacist.
If you have been recently hospitalized, bring the summary sheet of your hospitalization (if available).
Note the questions to ask your doctor in order not to miss during your visit.
If you have a form to fill out, go to the query tab. A fee will be charged depending on the complexity of the form.
Please bring your results with your blood pressure, your log book blood sugar levels capillaries, if your state requires it.
Please bring vaccination records of your child at each visit.
Arrive on time and for the children to arrive 10-15 minutes in advance.
If you cancel your appointment within 24 hours or less a$ 35 fee may be required
