To obtain a copy of your results, please refer to Carnet Santé Québec (https://carnetsante.gouv.qc.ca/). If you do not have access to your results in this way, you can call the clinic to activate your Patient Portal, a tool that is safe and reliable for the reception of your medical information. It is important to note that no document can be retrieved from your medical record without theexplicit authorization from your family doctor.

Forms to fill out

Before you submit any form, you must obligatory to fill in your party (personal information) and the sign (note that the forms voids will not be processed). You can submit the document to the front desk of the clinic or send it by email to adjointecmcsa@outlook.com (we do not accept that the PDF, not photographs). Charges may be applied, and the doctor has a legal period of 30 days to complete this form; you will be informed(e) when it is ready. Finally, please note that any subject other than the submission of forms sent to this e-mail will not be processed.

Renewal of the order
To renew your prescriptions, please contact your pharmacist to order it we send a renewal request by fax, or make an appointment at the clinic for re-evaluation.
Pharmacist
We work with pharmacists, Aylmer and Gatineau to coordinate your orders. For questions about the dosage or side effects, talk to your pharmacist, then book a follow up with us via the RVSQ or (819) 684-1234.

For a copy of the lost document, or a new query, please contact the secretariat at (819) 684-1234. A consultation is required for new requests. 

Referral to a specialist

To obtain a referral to a specialist or other health care professional, you should make an appointment with your family doctor to discuss the reference. The assessment of a doctor is required before providing any reference. If your condition changes or if the timeframe is exceeded, please make an appointment with your doctor for follow-up. The waiting time for an appointment with a specialist varies according to the specialist, the waiting list and the priority level.
